All requests to have a baby/child dedicated must come from the parent(s) or the legal guardian. Baby/Child dedication services will be scheduled with parents by phone once the request form has been submitted.

- As parents/guardians, do you believe that Jesus is your Savior and Lord?
- Have you dedicated yourself to the Lord to be a Christian Parent?
- Do you promise to pray for your baby/child, teach them the Word and lead your baby/child to love and serve the Lord Jesus Christ?
- Do you acknowledge that this baby/child belongs to God?
- Do you understand that this baby/child dedication is a public commitment to care for your baby/child’s spiritual needs as well as the physical and emotional needs?
- Do you understand that this baby dedication is not a baby/child baptism or a means of salvation for the child?
